The Bertram’s 24 hour nursing and care team aims to bring joy to each of our clients by fostering strong relationships with the client themselves and with family members. By maintaining open communication and conducting ongoing consultations with clients and their families, we tailor individualised care plans in order to create an environment of absolute comfort, safety and support for each client living with us at Bertram Cottage.
Clients with any level of care needs, including complex high care needs, are provided with support in their care and decision making, allowing them to age in place whilst still maintaining as much of their own independence as possible.
Dementia Care and Alzheimer’s Care
All nursing and care staff are qualified in Care and experienced with providing clients living with dementia support.
Structured and routine activities create a stress-free lifestyle and give clients living with memory loss and additional care needs a sense of satisfaction
Palliative Care
At Bertram Cottage we take a holistic approach to palliative and end of life care to provide comfort and support to both the client, their partner and family, with the goal of improving the quality of life for everyone involved.
An individual care plan is put in place to assist with the relief of pain and other symptoms (such as nausea or shortness of breath), as well as assisting with physical support, mental health and counselling, specialist care appointments and referrals, and tailored meal plans and exercise plans.
As a part of the palliative family care model, we also offer practical and emotional support to family members, as well as the peace of mind that their loved ones are receiving the best 24 hour care.
Complex Care
When a new client is welcomed at Bertram Cottage, we take the time to understand their life history, where they need assistance and support, and a personalised care plan is then tailored to their specific needs.
For clients who require a specialised level of care for an illness such as Cancer, Motor Neurone Disease, Dysphagia, stroke, depression or early onset dementia our team partner with the client and their family to ensure their care needs are catered for and the client feels safe and supported within Bertram Cottage family.
